Thank you for choosing Crestwood Medical Center for your healthcare needs.
We strive to verify your demographic and insurance information prior to your arrival. It is our goal to inform you of your estimated patient balance and financial responsibilities before your scheduled visit. All co-pays, deductible and estimated patient portions are due at time of service, unless other acceptable financial arrangements are made.
Crestwood Medical Center is considered a preferred provider and participates with several insurance companies and networks. As a courtesy we submit claims to most payers/insurance companies, regardless of our participation. A list of these networks contracted with Crestwood Medical Center may be provided to you upon request. However, this contractual relationship is no guarantee your claim will be paid at your maximum benefits. Some insurances change their participation with networks and facilities multiple times per year. We encourage you to contact your insurance company directly to ensure you receive the most current information regarding coverage, benefits, preferred providers and network participation.
We make every effort to provide our patients with an accurate estimate based on previous charge history and your benefit plan as described to us by your insurance carrier. We would like you to be aware that your hospital bill includes charges for hospital supplies, equipment and the services of our hospital staff. Physician fees for services will not be included in your hospital bill or estimate of balance due.
