Pre-Anesthesia Clinic 

• Please visit our Pre-Anesthesia Clinic or PAC prior to the day of your surgery.
   
• This appointment will be scheduled by your surgeon’s office. Please call 429-4960 or 429-5050 if you do not have an appointment. PAC Walk-in appointments are welcome. PAC is open from 7:00 a.m. until 5:30 p.m.; Monday – Friday.
   
• During PAC you will have all appropriate testing completed that will ensure your procedure is performed efficiently and effectively.
   
• We encourage that all pre-admission testing be completed 1-14 days prior to your procedure. You should allow for one hour to complete all pre-registration and PAC.
   
• Please bring a list of your regular medications, your dose and how often you take them. You may bring the bottles if you like. Go to the main entrance of Crestwood (Outpatient Center Entrance located on the North side of the facility) and the receptionist will guide you from there.
